Medical Review Process

Applying for residency at Williamsburg Landing includes a medical review. The purpose of the review is to assist Williamsburg Landing with its evaluation of the applicant’s ability to function at a safe and appropriate level necessary for independent living and how most effectively to achieve this.

1. Medical Screening
  • The applicant will forward the Personal Physician Statement form to their primary care physician with instructions to return the completed information to the Williamsburg Landing Clinic.
  • The Individual Statement of Medical History and Condition form completed by the applicant should be forwarded to the clinic for review by the Medical Director as soon as it is completed.
  • The history and physical information listed on the Personal Physician Statement forwarded from the primary care physician will be reviewed by the Medical Director.
  • An appointment will be scheduled with the Medical Director when all requested information has been received in the clinic to complete the admission process.

(If a prospective resident has not moved in within six months of the information provided then a review of current status may be required)

2. Functional and Cognitive Screening
  • This screening will be completed by clinic staff at the time of the appointment with the Medical Director. Three standardized tests recognized and validated by the medical community are administered to assist the Medical Director in assessing the applicant’s level of function and to provide Williamsburg Landing with data specific to the admission criteria.
  • The results of these screenings will be evaluated by the Medical Director.
